May 20th is a career holiday for traumatologists. On this day we have decided to talk about specific features of street medicine, when people with sprains, dislocations, and fractures come for a consultation in the Mobile Clinic.
Sometimes patients with various injuries are seen in the Mobile Clinic a few times in row during just one shift. All this is because life on the streets is linked to the increased risks of injuries. Elderly homeless people frequently get worse injuries because of their age features.
When a fracture of a dislocation is suspected, medical volunteers of the Charity Hospital issue a referral for a medical consultation in a specialized medical institution, so all the needed imaging techniques could be used. When possible, our specialists accompany a patient to an emergency room or follow him/her up afterwards. Advocacy is the most ffective way of protecting legal rights to health care for people without documents .
We are faced with a number of difficulties when treating dislocations and fractures in patients of the Charity Hospital. The main problem is that it is not possible for a patient to limit or completely stop physical activity. A homeless person is forced to move all the time to provide himself/herself with the basic things.
The second most important component and the key to success when treating injuries is proper rehabilitation, but for patients without documents it is not available.
A cast provides fixation in the fracture site for a callus to be formed, but in the conditions of living on the street it is not always convenient, as it softens in rain and frequently rubs the area. That is the reason why in these conditions our medical volunteers use various orthopedic products instead of a cast, but only in cases, when a type of an injury allows it.
